We host community workshops on important topics and local resources, available in person or via Zoom, to promote awareness and education among families.

We help families expand their knowledge and strategies in key areas such as:

-Self-Determination Program (SDP)
Individual Consultation and Guidance
“Parent-to-Parent Guidance”
We meet individually with parents to understand their concerns and needs related to their children
with disabilities. We collaborate with Regional Centers to provide ongoing support and follow-up
based on each family’s goals.
What is a PCP?
At Ochoa’s Consulting, we guide families through the creation of Person-Centered Plans (PCP)—a powerful tool that reflects the unique goals, capabilities, and needs of every individual and consumer. We meet with you in person, virtually, or by phone to thoroughly understand your history, challenges, and aspirations.
How we work in the process of creating a Traditional PCP?
- Initial Orientation to explain the transition
process. We provide an Agreement Contract
detailing how our team works. - Follow-up meetings to gather information from
the family and the consumer. - Development of the PCP (Person-Centered
Plan) and review with the family or consumer,
for collaborative submission to the Regional
Center. - We schedule a follow-up session with the
Regional Center to secure the individualized
budget. - If the budget is approved, the spending plan is
developed in collaboration with the family. - Resources and follow-up with Financial
Management Services (FMS) agencies. - We secure final confirmation from the FMS,
Regional Center, and family to ensure a smooth
start on the first day of the month.
Accelerated Transition Process
How we work in the Accelerated Transition Process?
- We provide a full explanation of our process
and workflow. This includes a Service
Agreement outlining the partnership between
the parent and the Ochoa's team. - We will meet with the family and the Regional
Center to review the services in the IPP and
establish a budget. - Follow-up meeting with the Regional Center to
obtain the budget. - Once the budget is agreed upon, we create the
spending plan in direct partnership with the
family. - Resources and follow-up with Financial
Management Services (FMS) agencies. - Confirmation with FMS, the Regional Center,
and the family that everything is ready to begin
on the first day of the month.
Independent Facilitation Services
- Train and guide families, consumers, and
Regional Center clients currently using traditional
services who decided to work with our team. - Assist in the resolution of situations identified
with the Financial Management Services (FMS)
agency, providers, and the Regional Center. - Train, advocate, and represent in the
application and acquisition of generic and
specialized resources (including IEP, SSI, Medi-
Cal, IHSS, and DOR). - Individualized (1:1) support for understanding
the budget and spending plans. - Ensuring a smooth transition and providing
support throughout the program's various phases. - Modify spending plans based on the client's
request. - Attending IPP and budget meetings alongside
the family and the Regional Center.
